Business Account Documentation Checklist

To open a business account, all owners, managers, officers, or authorized signers must be present. The following information is required and must be imaged prior to request for account review and approval through Compliance.

Business / Owner Required Information  
  • Verification of business‑in‑good standing from Secretary of State (Corporation, Limited Liability Company, Limited Partnership).
  • Provide the tax identification number (TIN/EIN) the business uses for reporting to the IRS.
  • Provide valid, unexpired, government‑issued photo identification for each owner, managing officer, or authorized signer
  • Business License or Business Tax Certificate
  • Business activity questionnaire with actual amounts (cannot have varies or 0)

Sole Proprietors
  • Current Business License
  • Fictitious Business Name Statement (if operating DBA)
Corporations  
  • Filed copy of Articles of Incorporation
  • Copy of Corporate Minutes or Corporate Resolution identifying:
    • the account is to be opened

    • signers to be on the account
  • Certified copy of Fictitious Business Name Statement (if operating DBA)
  • Certification of Beneficial Owners – Appendix A
Limited Liability Companies

Epic Business Checking and Epic Business Premium Checking accounts are subject to monthly maintenance fees of $15 and $19, respectively. Contact the credit union for current fee information.
